Injury Waiver

    In consideration of acceptance of this registration by the Markdale Golf & Curling Club ("The Club"), I (or my child, if participant is a minor) HEREBY RELEASE, WAIVE AND FOREVER DISCHARGE The Club and each of its members, officers and employees FROM ANY AND ALL claims, demands, damages, cost, expenses, actions and cause of action, whether in law or equally in respect of direct injury, loss or damage to myself (or my child, if participant is a minor) or property, arising or to arise by reason of myself (or my child, if participant is a minor) at The Club, that has not been contributed by any negligent act, by omission or commission, of any of the aforesaid.  By purchasing a membership or participating in a club event, league(s), general play, tournament or event, you grant The Club permission to use photos or videos taken during club events, tournaments, or general play for promotional purposes, including on our website, social media, and marketing materials.  If you prefer not to be photographed, please notify The Club in writing.



Rowens Law - Concusion Awareness

    Rowan’s Law – Concussion Safety Acknowledgement

    Under Ontario law, Markdale Golf & Curling Club cannot register any athlete under 26 years of age, or allow any coach/official to serve, unless they confirm annually that they have reviewed Ontario’s Concussion Awareness Resources and the Club Concussion Code of Conduct. 

    Per national and provincial rules, our club also extends this safety review requirement to all adult curlers. 

    Step 1: Review the Government of Ontario Concussion Resources
    Please review the official e-booklet matching the participant's age (link to e-books found under the Our Club, Our Links tab).

    Ages 10 and Under E-Booklet
    Ages 11 to 14 E-Booklet
    Ages 15 and Up E-Booklet

    Step 2: Review the Concussion Code of Conduct

    Please review our Concussion Code of Conduct, found under the Our Club, Rowens Law tab.

    Step 3: Mandatory Confirmation

    Resource Confirmation: I/We confirm that the participant (and parent/guardian if the participant is under 18) has reviewed the age-appropriate Ontario Concussion Awareness Resource within the past 12 months. 
    Code of Conduct Agreement: I/We confirm that I/we have reviewed, understand, and agree to abide by the rules and behavior expectations outlined in the Club's Concussion Code of Conduct. 
    Removal/Return Protocol Awareness: I/We acknowledge that if a concussion is suspected during any curling activity, the participant will be immediately removed from the ice and cannot return until they provide a signed medical clearance note from a doctor or nurse practitioner



Membership Selection Instructions

Select the membership fee option from the drop down menu in the section below and choose the league(s) you would like to join. 

Special instructions for setting up multiple Youths in the same family:
Set up the first youth’s information & select Youth Curling in the drop down menu below.  Repeat that process but when setting up the 2nd or 3rd+ youth from the same family, select the appropriate discount option in the Discount Option section of form.

Select Skip Choice League Fee if you are a member of a Skips Choice Team and the tax free fee will be added to your membership selection


Discount Options

The 2nd youth from the same family receives a 30% discount.  The 3rd (or more) youth from the same family receives a 60% discount.  Select the appropriate discount:
